We help families with case management and educate the public on how to plan for the needs of aging seniors.
We offer free of charge personalized senior placements, home care services, referrals services and elder care planning.
All the senior communities in our network are background checked and in good standing with appropriate state licensing. We also work with independent living communities to assist people with a limited income.

ELDER CARE PLANNING
We are pleased to provide this powerful planning tool with the elder resource planning for all the families to help assist seniors, their children and retirees. Our care managers will help you with the evaluation process, that will uncover the needs with care management, financial planning, end of life planning, Medicaid planning and insurance planning.
This elder planning resource is recommended for aging seniors and for those near or recently retired. We are gladly part of the San Francisco Bay Elder Planning Council.

HOMECARE SERVICES (NON MEDICAL)
Homecare services can enable them to stay in their own homes or live in a retirement community. Homecare aids can help with non- medical services. We work with our case managers and collect information about each client’s specific needs, develop solutions, and implement an action-oriented individualized care plan. Once this care plan is in action, the care managers regularly meet with clients and their support systems to ensure that each client receives the care needed to maintain their safety.
HOME HEALTH (MEDICAL REFERRALS)
We offer referrals for home health care working with accredited home health agencies. Our network of home health agencies accepts all types of health insurance.
This is medical care that's prescribed by your loved one's doctor. It requires a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to administer medication and skilled nursing care. The goal of home health care is to treat an illness or injury, to regain their independence and become as self-sufficient as possible.
Home health care can work in conjunction with other homecare aids who can assist with daily living activities, such as bathing and dressing. Also, home health providers specialized in wound care, injections and caregiver education.
