At Matching Communities, we had inspections all of them properly and work closely with many senior care communities to make sure all the regulations and standard are there. We believe in a hands-on approach to guided tours. We're committed to being with you every step of the way, ensuring that every resident receives the care they deserve while finding the perfect community. 4. Placement and Price Negotiation
When it's time to choose, we stand by your side to ensure a fair and negotiated pricing for you and your family. At discharge, we collaborate with discharge and medical professionals to coordinate all necessary paperwork, care plans, medication management, medical equipment, and transportation for a seamless transition in the moving process into your new home.
5. Follow ups & Welcome Gift
In our follow-ups and post placement process! We utilize a thoughtful 30/60 approach, which includes follow-ups at 48 hours, 14 days, 30 days, and 60 days after placement. Our goal is to ensure that everyone is happy and comfortable in their new home. As a special touch, we also bring a warm welcome gift for your loved one to show how much we care.

COMPREHENSIVE HOMECARE SERVICES
Welcome to our Homecare Services, where we empower seniors to remain in the comfort of their own homes or transition smoothly into retirement communities. Our Personal Care Assistance offers non-medical support tailored to individual needs, ensuring compassionate care every step of the way. Our dedicated case managers assess each client’s specific requirements, creating and implementing personalized care plans. We continuously monitor progress and engage with clients and their support systems to guarantee the highest level of safety and well-being.
HOME HEALTH & HOSPICE REFERRALS
We offer referrals for home health care working with accredited home health agencies. Our network of home health agencies accepts all types of health insurance.
This is medical care that's prescribed by your loved one's doctor. It requires a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to administer medication and skilled nursing care. The goal of home health care is to treat an illness or injury, to regain their independence and become as self-sufficient as possible.
Home health care can work in conjunction with other homecare aids who can assist with daily living activities, such as bathing and dressing. Also, home health providers specialized in wound care, injections and caregiver education.
