Senior Referral Services
Homecare Services (Non-medical)
Homecare services can enable them to stay in their own homes or live in a retirement community. Homecare aids can help with non- medical services. They can help when there are physical or cognitive limitations, such as helping your parents to get around, reminding them when it’s time to take a medication, and ensuring their safety.
We work with accredited care agencies, and collect information about each client’s specific needs, develop solutions, and implement an action-oriented individualized care plan. Once this care plan is in action, the care managers regularly meet with clients and their support systems to ensure that each client receives the care needed to maintain their safety.

Home Health Care (Medical)
We offer referrals for home health care working with accredited home health agencies. Our network of home health agencies accepts all types of health insurance.
This is medical care that's prescribed by your loved one's doctor. It requires a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to administer medication and skilled nursing care. The goal of home health care is to treat an illness or injury, to regain their independence and become as self-sufficient as possible.
Home health care can work in conjunction with other homecare aids who can assist with daily living activities, such as bathing and dressing. Also, home health providers specialized in wound care, injections and caregiver education.
Hospice Palliative Care
Hospice care is a type of health care that focuses on the palliation of a terminally ill patient's pain and symptoms and attending to their emotional and spiritual needs at the end of life.
Hospice provides attentive medical, emotional, spiritual, and practical services for those with life-threatening illnesses.
The goal is to enable patients to be comfortable and free of pain, so that they live each day as fully as possible.
Palliative Care-also called comfort care, is about providing relief. It is administered to people who have a debilitating disease, whether or not they are on their way to being cured, and no matter how long their lives will be lived.
“You really can change the world if you care enough.” Marian Wright Edelman
